Information for Current International Students
Maintaining F-1 Status: Refer to this form if you have any questions concerning how to maintain your F-1 status.
Request for Letter of Full-time Verification: This form is to be filled out when a student asks for verification of enrollment, letter for social security, letter for driver's license, or travel signature for their I-20.
Change of Address, Major, and Name Form: This form is to be used when a student needs to change their Address, Major, and or Legal Name. F-1 students are required by immigration to report to the Office of Admissions and Records these changes within 10 days of the change. After the request has been processed, the form will be placed at the front window for students to pickup to place with their important immigration documents.
Program Extension Request Form: All F-1 students have an end date listed on their I-20. If you have good reasons to extend your study, you may do so by filling this form out and meeting with an Advisor. AFTER you have met with an advisor you should make an appointment with Dilek Arig--darig@mc3.edu, 215-619-7444--to complete the extension process. Students will need to provide additional financial documentation for another year of school in order to be approved.
F-1 Work Options: An F-1 student is only allowed to work on campus 20 hours a week. Please refer to this form as to what other options, you may have as a F-1 student.

Health Insurance Waiver Form: This form is used by students to waive the school's health insurance in each semester. A copy of your insurance policy must accompany this form. An insurance card is not acceptable.
International Student Exit Form: This form must be filled by any F-1 student planning to leave MCCC permanently. The following reasons include
- Graduation
- Return Home
- Transfer
- Applying for OPT
- Changing their Status to something other than F-1
